When Surgeons Struggle: The Impact of Alcohol Dependency in the Operating Room

A concerning study published in Archives of Surgery revealed that approximately 15 percent of licensed surgeons showed signs of alcohol-related problems.

Female surgeons reported these issues at a significantly higher rate than male surgeons. Despite this, surgeons in the United States are not subject to routine alcohol or substance abuse screenings, unlike professionals in other high-risk fields.

“Patients trust that their surgeon is fully capable of performing their procedure safely,” said CHH Partner Jim Harman. “When substance abuse goes unchecked in the medical field, that trust is broken, and the consequences can be life-threatening.”

The demanding nature of surgical work plays a role. Long hours, high stress, and intense emotional pressure can create an environment where alcohol dependency develops quietly and goes unnoticed. Hospitals and medical organizations emphasize patient safety, but protecting patients also requires supporting the mental and physical wellbeing of the providers themselves.

A survey was sent to 25,000 licensed surgeons. Approximately 1,112 of the 7,197 responding surgeons provided answers indicating that they have an alcohol dependence or abuse problem. Twenty-six percent of women who responded to the questionnaire had alcohol dependency issues while among men the figure was a lower 14 percent.

Routine screenings, confidential support systems, and mental health resources could meaningfully reduce risks for patients and surgeons alike.

Alcohol dependency can impair judgment, slow reaction times, and increase the likelihood of avoidable surgical errors. For families facing the aftermath of a surgical mistake, understanding whether impairment was involved is critical.
